"On this 8th day of May, A.D. one thousand eight hundred and seventy-five, personally appeared before me, Clerk of the County Court and court of record within and for the county and state aforesaid, David Salisbury, aged 76 years, a resident of the City of Auburn, county of Cayuga, State of New York, who, being duly sworn according to law, declares that he is the identical David Salisbury who was enrolled on the 6th day of Sept, 1862, in company F or the 160th regiment of NYVols commanded by Col Chas C. Dwight, and was honorably discharged…he contracted disease by which he was greatly disabled…that his present condition is such that he cannot…since leaving the service this applicant has resided in the City of Auburn in the State of New York, and his occupation has been that of a no occupation. That prior to his entry into the service above named he was a man of good, sound, physical health, being when enrolled a Cooper. That he is now wholy disabled from obtaining his subsistence by reason of…in degree equal to loss of two hands or two feet and requiring constant aid and attendance of another person"...witnessed by Oliver Wood and Henry D.
